Investigate and prepare Workers' Compensation claim for reference to a Medical Board. Solicitor only. This is a maximum fee grant of aid (including agent's fees). Claims can be made up to the maximum fees in accordance with the attached proforma account. The following items are attached and/or available in the Forms Section of Grants On-Line via the Internet: Item 1. Authority to Workers' Compensation Board. Item 2. Authority to Solicitors. This grant of aid will only become effective upon your client completing the above authorities. Please send signed: 1. Original of item 1 to Workers' Compensation Board. 2. Copy of items 1&2 to Legal Aid Queensland. Reasonable outlays will be paid. The maximum fee payable on this invoice for disbursements is $250.00.
